Join us for cocktails, dinner, and award presentations. 

6:30pm - 7:00pm Cocktails 

7:00pm - 10:00pm Dinner and Awards 

During registration, please indicate if you have any dietary or allergy restrictions. You will also have the opportunity to select "Vegetarian" or "Non-Vegetarian" dinner options. 

Registration fees are to be paid by Credit Card at the end of this registration form.
